Checklists for Parents
Simple checklists to help you organise what you are noticing.
These checklists help parents organise concerns before a school meeting, a clinic enquiry or a first paediatric visit. They are educational tools, not diagnostic questionnaires — no single item confirms or rules out anything. Similar observations can have very different explanations, and are best considered in the wider context of your child's life.
Before you send an enquiry
A short organising list to help you prepare what to share with the clinic team.
- Write a one-page summary of your main concerns.
- Note when concerns started and how they have changed over time.
- Gather recent school reports, therapy notes and any previous assessments.
- Note your child's languages, health background and current medications.
- Write down two or three specific questions you'd like answered.
Observations to note over two weeks
If a concern feels unclear, brief daily notes over a fortnight often help.
- What was the situation? (setting, time of day, who was there)
- What did you notice? (behaviour, communication, feelings)
- What worked, and what did not?
- How did your child recover afterwards?
- Any patterns across sleep, food, routines or transitions.
For a school meeting
A brief structure that keeps parent-teacher meetings focused and useful.
- Your top two or three concerns, in writing.
- Recent examples with dates where possible.
- Specific questions for the teacher and learning support staff.
- Any previous reports you would like the school to consider.
- Agreement on next small step and a review date.
When to seek urgent help
These situations need immediate attention, not an enquiry form.
- Any concern about self-harm, safety or thoughts of harming self or others.
- Sudden serious change in behaviour, alertness or mood.
- Medical emergencies, injuries or acute physical symptoms.
